As I remember the book the tension was in the realization that his mental process and abilities were going to decline and he would not even remember the intelligent manner in which he was working, acting and living.
In that long ago time the book was rather popular.

Flowers for Algernon is a great book, read it maybe 40 or 50 years ago.
